For 2025 coverage, Ross Center for Nursing and Rehabilitation will not automatically mail Form 1095-C to employees and other covered individuals. A copy of your Form 1095-C is available to you upon request at no charge. To request a copy or ask questions about your Form 1095-C, please contact Human Resources:
